Lakeside Home Services

When our previous dishwasher died two weeks before Christmas, we bought another one. The vendor set us up with Lakeside Home Services to install the new dishwasher for $150+tax plus $40+tax to remove and dispose of the old dishwasher.
I suspect the old dishwasher acted like a lemon because it had problems with the subfloor being uneven, so I purchased a piece of plywood to replace the subfloor.
When the installers arrived, I told them I would replace the subfloor once they removed the old dishwasher. The lead installer said he would do it for me.
Error #1: I believe him. Instead, he just installed the new subfloor over the old subfloor.
The floor was now even but the height was seriously reduced. The installers said they would make it work by adjusting the feet.
Error #2: I believe them and didn't hover. I didn't see them shove the dishwasher into the spot.
They showed me the dishwasher was working, I paid them and signed the paperwork and they left.
Error #3: I did not fully inspect the work until after they left. The insulation was all bunched up at the front. See images below.
I called the company's office and requested they come back at a mutually agreed date and remove the dishwasher so that I could remove the old subfloor.
Christmas came and went.
The Friday before they were to return, the company tells me it was my fault, the site wasn't properly prepared and I have to pre-pay $120+tax for them to return. Just wanting the job completed, I paid.
They day of their return, they were to arrive between 08h30 and 09h30. After several phone calls and e-mails, they finally arrived at 09h40.
They removed the dishwasher and the new subfloor.
At 09h50 I removed the old subfloor, vacuumed out the space, and reinstalled the new subfloor.
At 09h55 they began reinstalling the dishwasher, this time without damaging the insulation.
By 10h14 they were gone, no paperwork, no signature.
